About the Program
Our Pivot Your Diabetes program is designed to equip you with the knowledge and tools needed to effectively manage Type 2 Diabetes (T2DM).
Whether you’re newly diagnosed or have been living with diabetes for years, this program aims to enhance your quality of life through expert guidance and practical strategies.
Led by a team of qualified professionals—including Accredited Practising Dietitians who are also Credentialed Diabetes Educators (CDEs), a Physiotherapist, and a Pharmacist—you’ll receive personalised, evidence-based care every step of the way.
What Does the Program Involve?
This 8-week program includes:
- An initial one-on-one appointment to evaluate your current diabetes management and confirm program suitability.
- Eight weekly group sessions lasting 1.5–2 hours, covering key topics tailored to help you better understand and manage your diabetes.

Who Can Join?
You are eligible for the Pivot your Diabetes if you:
- Have Type 2 Diabetes (T2DM), regardless of how long you've had the condition.
- Have a current Care Plan (Item 721).
- Are referred by your GP under Medicare’s Group Allied Health Services for patients with Type 2 Diabetes.
How much does it cost?
Initial Assessment: $90.00 with a $77.40 Medicare rebate (out-of-pocket cost $12.60).
Per Group Session: $30 with $19.30 Medicare rebate (out-of-pocket cost $10.70).
Bulk billing is available to eligible clients; please speak to our reception staff for more information.
